PROCEDURE
ON YOUR FIRST VISIT YOUR HEALTH HISTORY will be collected to provide your therapist with any previous conditions that you have had, any medications that you are on and what they are treating, and your current health status. You and your therapist will go over your health history form & discuss any concerns you may have prior to your treatment. This process helps your therapist become more familiar with you & your needs and will help your therapist provide you with the best care possible that is safe & effective.
IMPORTANCE OF YOUR HEALTH HISTORY form requires that your therapist keep this information current; which is why your health history form needs to be updated yearly. When that time comes, your therapist will sit down with you and go over any changes to your health status.
PLAN TO ARRIVE A FEW MINUTES EARLY FOR YOUR FIRST SESSION as it usually takes about 10 minutes to fill out your health history form. After the HH form has been filled out, your therapist will take a few minutes to go over your form with you, and then perform an assessment of your physical condition. The assessment is a vital part of your treatment and helps the therapist determine what type of treatment is most appropriate.
SUBSEQUENT VISITS will start with a brief assessment so that we can follow the changes in your condition & make sure your treatment is on the right track!
PRIVACY
PROTECTING YOUR INFORMATION is a priority that your therapist takes seriously. Any information that is collected from you: your name, address, phone number, etc. will be retained for only a period specified by law, and will only be used for the purpose it was intended. Storage, retention, and destruction of your personal information comply with existing legislation, standards of our regulatory body, the College of Massage Therapists of Ontario, and the law.
RELEASING INFORMATION about your health history or any other information collected by your therapist cannot occur without your knowledge. Your therapist will obtain your consent to release any information that may be requested by your medical physician or to any other health care practitioner. Your information may be accessed by regulatory authorities under the terms of the RHPA (regulated health professions act) for the purposes of the College of Massage Therapists of Ontario, fulfilling its mandate under the RHPA, and for the defense of a legal issue.
TRANSPARENCY: your therapist will provide any information that is retained about you at your request.
DRESSING AND UNDRESSING. Your therapist will leave the treatment room to allow you to dress and undress in private. You may leave on any article of clothing that you wish to your comfort level. When your therapist returns to the room, she will knock on the door and announce herself before reentering the room.

OBTAINING YOUR CONSENT: prior to each treatment your therapist will provide you with information about your treatment plan for the day. This will include techniques to be used, the purpose of those techniques, draping procedures pertaining to your treatment, pillowing, general risks & benefits of massage therapy & specific to any advanced techniques that may be used, etc.
If you have any questions, feel free to ask them at any time! Your therapist wants to ensure that you are fully informed about what to expect and can provide informed consent to your treatment.
